    Questionnaire Templates

    Within Atlas, a Templates page houses all of the standard templates and custom questionnaires. Atlas offers 20+ out-of-the-box templates (as listed below), but you can additionally upload an excel template, use the provided template, or create a custom questionnaire. 

    Once you land on the Template page, you should see all of the questionnaires that have been created in different states: Draft, Published, and the 20+ standard out-of-the-box templates.

    The following actions are available: 

    • Sorting: Template Name, Date Modified, State
    • Draft questionnaires: Preview, Rename, Edit, Duplicate, Delete
    • Published questionnaires: Preview, Send, Rename, Delete 

    Published questionnaires are the ones that will be available as options in the questionnaire list when ready to send out to recipients. Only Published questionnaire states will be able to send out. If you need to remove the questionnaire from the list of options to choose from, you can delete the template. If you have published a template and want to make some changes, you can unpublish the template if you didn’t send a request yet. If you have sent a request already, feel free to duplicate the template and edit the copy.
