When your company is asked to prove its security, prospects and customers send you security questionnaires to fill out. This article covers how questionnaires get into Titan and how to navigate the Questionnaires page. When you're ready to answer, see Responding to security questionnaires in Titan Assess.
A quick note on terminology: a questionnaire is the security form you import, complete, and return to the customer who sent it. If instead you want to send questionnaires to evaluate your own vendors, that's called an assessment. See Create and send an assessment in Titan Assess.
Prerequisites
Before you begin, make sure you have access to the questionnaire file or link.
How a questionnaire gets into Titan
There are two ways a questionnaire can show up on your Questionnaires page:
- A customer sends you one: you receive an email with a link.
- You upload a file or paste a web portal URL using New questionnaire.
Open a questionnaire sent to you
Most questionnaires arrive by email invitation. Clicking the link opens the questionnaire and adds it to your Questionnaires page; there's nothing to upload. If you received one as a file or a portal link instead, start a new questionnaire below.
Start a new questionnaire
To begin:
- Go to My Company > Questionnaires.
- Click New questionnaire.
- Choose Upload a file (.xlsx, .docx, .pdf) or Paste a URL to a web portal (for example, OneTrust).
Titan Assess then guides you through the matching submission flow.
Option 1: Upload a file
After uploading, choose one of three methods:
- Full Service (PDF, DOCX, XLSX)
- Auto Import (DOCX, XLSX)
- Self Import (DOCX, XLSX)
Full Service
The SecurityScorecard team imports, reviews, and autofills your questionnaire for you. Best for complex files, tight deadlines, or minimal manual effort.
- Upload your file and choose Full Service.
- Fill in:
- Questionnaire name (auto-fills from the file name; editable).
- Customer name (existing or new).
- Owner(s), who receive notifications and updates.
- SLA, the turnaround time your plan allows.
- Segment, if applicable.
- Select Show advanced options for a due date, expiration date, revenue figure, or notes.
- Select Submit questionnaire.
You get an email once your autofilled file is ready to review.
Auto Import
Auto Import uses AI to detect and map question-and-answer pairs. It's the fastest option, usually finishing in minutes, and works best for standard-formatted files.
- Upload your .xlsx or .docx file and choose Auto Import.
- Fill in the same details as Full Service above (questionnaire name, customer, owner, SLA, and segment), then use Show advanced options for the optional fields.
- Click Submit questionnaire to begin the AI import.
- Review the imported question-and-answer preview. Titan Assess extracts questions and answers from your file, organizes them into sections based on the file's layout, and pre-maps them for autofill.
- Make edits using:
- Bulk Select: Select multiple question-and-answer pairs at once to include.
- Single Select: Tell the tool where specific data is located (for example, Question = Column A).
- Titan Agent: Type a prompt, such as "Remove Column D from this selection."
- Once everything looks correct, click Submit for autofill.
Titan Assess autofills your questionnaire from your Knowledge Base. Review and approve the answers, then download or submit the final file.
Self Import
You map the questions and answers yourself; Titan Assess then autofills them from your Knowledge Base. Best for non-standard file layouts.
- Upload your .xlsx or .docx file and choose Self Import.
- Fill in the same details as Full Service above (questionnaire name, customer, owner, SLA, and segment).
- Map your file: tell Titan Assess where the questions and answers are located (for example, which columns or rows contain each).
- Click Submit for autofill.
Titan Assess autofills the imported questions from your Knowledge Base. Review and approve the answers, then download or submit the final file.
If your questionnaire is in another language, select This questionnaire is in a foreign language. Work on it in English, and Titan Assess translates your answers back when you export.
Option 2: Submit via web portal
Some questionnaires live in a web portal (such as OneTrust) rather than in a file. If the portal lets you export it, download it and use Option 1: Upload a file instead. Otherwise, give Titan Assess the portal URL and your login credentials, and it accesses the portal on your behalf and autofills answers from your Knowledge Base.
- On the New questionnaire screen, choose Paste a URL.
- Paste the questionnaire portal URL.
- Click Next.
- Enter your username and password, or toggle No password needed if the portal doesn't require one. Then fill in the same details as Full Service above (name, customer, owner, segment). SLA here is 1-3 business days, based on your plan.
- Click Show advanced options for the optional fields.
- Click Submit questionnaire.
You see a confirmation message that your request was submitted.
Note: Your URL is kept secure and used only to autofill responses on your behalf.
Understand the Questionnaires page
Your Questionnaires page organizes questionnaires into tabs by where they are in the process: Active (in progress), Completed (finished), Submitted (importing or autofilling), and Requests (awaiting approval before import).
Within a tab, questionnaires may be grouped by their current state, for example, Ready to review, and you can use the search box to find a specific one. Each row shows its Status (a progress bar), the Customer it's associated with, its Owner (the responsible team member), its Submitter (or "No submitter" if none is set), and any Revenue value entered (otherwise "N/A").
Next steps
When you submit a questionnaire for Full Service or via a web portal, it appears in the Submitted tab while it imports and autofills. Once quality-assured, the SecurityScorecard team emails the Owner(s) a link to the pre-filled questionnaire to review. See Responding to security questionnaires in Titan Assess.
